Qualifications for Application

| 1. Must be at least 18 year of age. 2. Complete the application and return within 10 business days with the following documentation: |

| a. State Birth Certificate for proof of age; b. High School grade transcript or GED certificate and official report of test results. c. Social Security Card d. Drivers License or Picture I.D. e. Military transfer or discharge Form DD-214, if applicable. |

| 3. Appear for Mechanical Aptitude Test when notified. Notification is sent by Certified Mail. 4. Appear for interview when notified. Notification is sent by Certified Mail. |

| The Piping Industry Training Center Joint Apprentice and Training Committee takes affirmative action to provide equal opportunity in apprenticeship.
